If right and wrong are in the eye of the beholder, how can someone "know" the difference?

does "knowing the difference between right and wrong" just mean "having the conventional idea of what is right and wrong"? if that's the case, then obviously someone who commits a crime doesn't know the difference between right and wrong. anyone who's ever done anything wrong should be labeled criminally insane
