What is the TRUE plot of shutter island?

He is actually insane. He hallucinates that he's a cop investigating Shutter Island so they all go along with it until the end when it becomes obvious that he is insane. To me, it almost seemed like a "role-playing" that the staff on the island did with him commonly (whenever his memory lapsed/he hallucinated again)....
